To create a CVI:

Create - Click Create in the upper right corner, hover over CVI and select the type of CVI you will be creating: Equine, Companion Animal, Production Animal or General CVI.

Ownership - Select Consignor/Owner: Create a new Consignor/Owner by selecting Create New, or enter the name of an existing contact into the search box and select that contact from the list.

Ownership - Select Consignee/Recipient: Create a new Consignee by selecting Create New, or enter the name of an existing contact into the search box and select that contact from the list.

Movement - Select Origin and Destination: Both will default to Same as Consignor/Owner and Same as Consignee/Recipient, respectively. If you need a different address/contact for either of these fields, uncheck the box and search for or create a new contact.

Movement - Select Purpose of Movement: choose the purpose of movement from the list.

Movement - Shipping Date: Optional field where you can specify a specific shipping date.

Movement - Carrier Type and Carrier: Select the Carrier Type from the list. Select Same As Consignor or Consignee if the respective parties are transporting the animals. If they are not, select Other to search for an existing contact or create a new one.

Animals - Inspection Date: Select the inspection date from the calendar.

Animals - Species: Select the Species from the drop down menu. Species Type can be identified to further specify the species information.

Animals - Select the animals from the list of animals assign to the Consignor/Owner or add new animals by clicking Create New. When creating an animal or animal group, complete all of the red asterisked fields and click Save.

Animals - Add Animal IDs: Under IDs, click Add an ID. Enter the Type of ID in the field or select from the list of IDs. Enter the ID number in the Enter an ID field, below the type of ID. To add an additional ID, click Add an ID again.

Animals - Add Tests/Vaccinations/Treatments: After selecting an animal, test, vaccinations and treatments can be added to the certificate. If you have selected an existing animal with completed tests, vaccinations or treatments, they will show in the animal profile. To include them on the CVI, check the boxes to the left of the test, vaccination or treatment. To add new tests, vaccinations or treatments, click the Add button to the right of the Tests, Vaccinations and Treatments headers. The red asterisked fields are required. Click Save to save and the information you have entered will automatically be included on the certificate.

Permit - Permit Number may be required, depending on the state to which the animals are traveling. Consult www.animalregs.com (found under Help on the left-hand side) or the state of destination to learn if a permit number is required.

Remarks - Add Remarks: The system is loaded with a list of Common Remarks, as well as species-specific remarks. You can also click Create New to add a custom remark.

Area Status - Production Animal and General CVI only - optional Area Status information can be added.

Herd/Flock - Production Animal and General CVI only - optional herd/flock health information can be added.

Preview and Sign: Click Preview & sign in the upper right-hand corner to review the information entered on the CVI. You can preview the PDF or edit the CVI if changes need to be made. Once a certificate has been signed, it cannot be edited. After you have previewed to ensure the certificate is correct, click the Sign button, enter your username and password, and click Sign.

Owner Access of Certificates: Owners will be able to access completed certificates through their MyVetLINK account. To print a copy of the certificate, click Show PDF to view and print the certificate through Adobe Reader.
